Continuous veno-venous hemofiltration in patients with metformin associated lactic acidosis

چکیده
Introduction Metformin is frequently used by type II diabetics. A rare complication of its use is metformin associated lactic acidosis (MALA). This condition is often accompanied by extreme metabolic acidosis and hemodynamic instability for which ICU treatment is mandatory. Guidelines advice intermittent hemodialysis for correction of metabolic acidosis and to clear metformin. However, haemodynamic instability may prefer continuous veno-venous hemofiltration (CVVH). We evaluated the effect of CVVH in critically ill patients admitted with MALA.

Severe lactic acidosis and rhabdomyolysis following metformin and ramipril overdose.
We report the case of a 46-yr-old male who developed severe lactic acidosis, cardiorespiratory arrest, and rhabdomyolysis following an overdose of metformin and ramipril. The lactic acidosis was successfully treated with early high-volume continuous veno-venous haemofiltration. Rhabdomyolysis and lower limb compartment syndrome developed later. The patient otherwise made a good recovery. We dis...
متن کاملEffectiveness of Continuous Veno-Venous Hemofiltration and Intermittent Hemodialysis in the Treatment of Severe Acute Phenobarbital Poisoning
Background: Phenobarbital poisoning is common in Vietnam. The aim of this study was to compare the effectiveness of continuous veno-venous hemofiltration (CVVH) and hemodialysis (HD) on clinical outcomes in the treatment of severe acute phenobarbital poisoning. Methods: This was a retrospective observational historically controlled study. 42 patients with severe phenobarbital poisoning were enr...
متن کاملReality of severe metformin-induced lactic acidosis in the absence of chronic renal impairment.
BACKGROUND Lactic acidosis in metformin use is a widely recognised but rare side effect. Case reports usually describe elderly patients with conditions which in themselves can cause lactic acidosis or with known contraindications to metformin. We present cases of an elderly woman, a younger woman and a man who developed serious metformin-induced lactic acidosis in the absence of chronic renal i...
متن کاملExtreme lactic acidosis type B associated with metformin treatment
The elimination of metformin is exclusively through the kidneys and elevated plasma concentrations can cause lactic acidosis. We report a case of severe lactic acidosis (pH 6.60) occuring with ostensibly normal therapeutic doses of metformin in the setting of acute renal failure. Continuous veno-venous haemodiafiltration decreased plasma metformin concentrations from 266 lmol/L at presentation ...
متن کاملSuccessful management of high-dose metformin intoxication. Role of vasopressin in the management of severe lactic acidosis.
On induction of anesthesia (for intubation), the patient hemodynamics became very unstable prompting the initiation of both Epinephrine and Nor-Epinephrine Intra-venous (IV) infusions. Glucose 10% IV infusion and Insulin IV infusion were also started. The pre-transfer management also included Hydrocortisone IV justified by the marked hemodynamic instability and mediocre response to inotropic su...
